Até o final do primeiro semestre de 2008, criticar as multinacionais era considerado anacronismo, coisa de militante petista, retrógrado e sem valor.
Tudo era globalização, o mundo pertencia as grandes empresas, fazer uma máquina na China ou Cingapura era considerado modernidade.
O mundo privado era divino e responsável, o mercado se regulava e reclamar era coisa de gente chata.
Mesmo assim, empresas como a BMW, insistiam em manter a fabricação de suas máquinas sob o rígido controle de qualidade alemão.
Outras, eram obrigadas a recolher milhares de baterias feitas fora do Japão.
Controle de qualidade não é uma característica de um país em especial, mas de uma empresa especificamente.

Algumas grandes empresas de ultra-som, tratavam o mercado brasileiro como de terceira categoria. Tudo era terceirizado, o lucro vinha antes da imagem da empresa.
Mas era feio reclamar, só mesmo um blog ranzinza como o ECHOTALK para insistir nesse assunto…

Estamos em Março de 2009, soubemos da pior forma que a busca irresponsável do lucro levou a quebra geral do setor privado.
Não fosse o socorro com dinheiro público, o caos seria total.

O blog continua a receber histórias e mais histórias de descaso com os compradores de máquinas. Quebras precoces, demora em corrigir erros de fábrica, custo absurdo de manutenção…
.
Fabricamos agora os aparelhos na China, eles quebram antes de 6 meses, mas tudo bem.
No DIC 2009 teremos simpósios satélites luxuosos, das mesmas empresas que destratam o ecocardiografista. Vários negócios serão realizados em Fortaleza, promessas de imagens superiores e ausência de quebras serão feitas entre cafés e presenças ilustres.
Promessas que não serão cumpridas, como nunca foram.

Impact of diabetes on high-sensitive C-reactive protein in patients with acute and chronic coronary syndromes.

Introduction
Current evidence has implicated the role of inflammation in atherogenesis and plaque destabilization. Inflammatory cytokines may attenuate interstitial collagen synthesis, increase matrix degradation, and promote apoptosis in several atheroma-associated cell types, and all these cellular events may enhance plaque vulnerability. Hyperglycemia per se is a risk factor for poor outcome after myocardial infarction. An enhanced inflammatory state is probably related to a worst cardiovascular prognosis in type 2 diabetic patients.
Aims and Methods: To study the influence of the diabetic state on inflammation in acute and chronic coronary syndrome (ACS and CCS, respectively). High-sensitive C-reactive protein (hsCRP), a surrogate marker of inflammation, were measured in 74 type 2 diabetic subjects (38 with non ST-elevation myocardial infarction and 36 with stable angina), and 38 non-diabetic subjects (22 with non ST-elevation myocardial infarction and 16 with stable angina).The inclusion criteria for ACS were: 2 episodes of resting angina or 1 episode of more than 15 minutes in a 24 h period without elevation of ST-segment or with T wave inversion in the ECG. hs-CRP was measured by nephelometry.
Results: In the ACS group, hsCRP was 2.6 times greater in the diabetic group in comparison to non-diabetic subjects [4.75(0.97) vs. 1.83(0.77) mg/L, p<0.01]. In the CCS group, hsCRP was greater in diabetics [1.75 mg/L (0.22) vs. 0.55 mg/L (0.18) mg/L, p<0.05]. Hence, hsCRP is similar in T2DM with CCS to non-DM with ACS [1.75 mg/L (0.38) vs. 1,83 mg/L (0.46) mg/L].
Conclusions: Elevated plasma concentrations of the inflammatory biomarker high-sensitivity C-reactive protein is observed in diabetic patients in both acute and chronic coronary syndromes. The diabetic-related amplification of inflammation in acute and chronic conditions could be related with the worst prognosis of coronary disease in T2DM.

Carotid plaque is more frequent, but not related to intima-media tickness in asymptomatic type 2 diabetic subjects.

Introduction
Atherosclerosis develops over the course of several years, beginning in the early teenage years, and accelerating in the presence of type 2 diabetes. This process occurs due to a lipid retention, oxidation, and modification, which provoke chronic inflammation at susceptible sites in the walls of all major conduit arteries, including the carotids. Ultrasound measurements of intima-media thickness (IMT) and plaque occurrence in the carotid arteries are important not only for the assessment of structural alterations but also because the extent of atherosclerosis in these vessels reflects the severity of arterial damage in other vascular territories.
Aims and Methods: To describe the findings of a carotid artery study including the measurement of IMT, the presence of atherosclerotic plaque, and their correlation with the presence of type 2 diabetes in asymptomatic subjects.
Two hundred ninety two patients (220 non-diabetics and 72 Type 2 diabetics; age 55(8) years) selected from Metabolic Syndrome outpatient clinic of University of Campinas Medical Hospital who underwent a carotid artery ultrasound in our Doppler vascular laboratory were screened for carotid plaque and IMT measurement. Type 2 diabetics (DM) and Non-diabetics (nDM) groups showed no differences in age: 56(8) vs. 54(9) y, gender (male: 62 vs. 66%), presence of hypertension (88 vs. 84%), dyslipidemia (66 vs. 58%), smoking (15 vs. 20%), use of ACE inhibitors (88 vs. 78%). Diabetics subjects were taking aspirin and statins in 98 and 82%, respectively, in comparison to 1 and 42% in nDM group (p 2.0 mm that did not uniformly involve the whole left or right carotid artery with or without flow disturbance.
Results: IMT was greater in DM group [1.014(0.018) vs. 0.690(0.006) mm, p<0.001]. The presence of atherosclerotic plaques was 19.4% in DM, and 1.4% in nonDM asymptomatic subjects (p<0.001). In the DM group, patients with plaques showed similar IMT measurements to patients without plaques [1.030(0.038) vs. 1.009(0.020) mm]
Conclusions: Assessment of carotid arteries identified increased incidence of atherosclerotic plaque in Type 2 diabetic patients without symptomatic CAD. IMT was increased in diabetics independent of the presence of the plaque.

The gold standard for noninvasive imaging in congenital heart disease: echocardiography

Purpose of review: Echocardiography in pediatric and congenital heart disease is a key diagnostic technique in patients with congenital heart disease. Due to new technological developments, it has become a rapidly evolving field.
Recent findings: In this review, we focus on recent developments in standardization and validation of standard techniques in pediatric and congenital echocardiography. This is mainly related to standardization of image acquisition and normalization of measurements for body size. The rest of the review is focused on the application of three-dimensional echocardiography, tissue Doppler imaging and Speqle tracking techniques to pediatric heart disease.
Summary: New developments in standardization of echocardiography, the introduction of three-dimensional echocardiography and new functional techniques such as tissue Doppler and Speqle tracking strengthen the position of pediatric echocardiography as the most important diagnostic tool for patients with congenital heart disease.

Apesar da maioria dos leitores do blog não se dedicarem ao eco infantil, existe farta documentação que coloca o método como absoluto.
No Brasil, temos expoentes na área, algums já estabelecidas, outros em evolução.
Devo ter feito uns 5 000 exames em crianças, que agora são raros pra mim. Foi muito importante por um tempo, permitiu que eu fosse um ecocardiografista difícil de substituir.
E ser difícil de substituir, lembrando que insubstituível ninguém é, é a melhor garantia de emprego no meio médico.
