In a study of 2D strain imaging, a peak systolic longitudinal SR of −0.83 s⁻¹ and an early diastolic SR of 0.96 s⁻¹ predicted significant (>70%) stenosis with a sensitivity and specificity of 85% and 64% and 77% and 93%, respectively,²⁶ suggesting the potential for early diastolic deformation to improve diagnostic accuracy. In another study of 108 patents undergoing coronary angiography, a 2D longitudinal strain of −17.9% discriminated severe 3-vessel or left main disease from lesser coronary artery disease with a sensitivity and specificity of 79% and 79%, respectively.²⁷ Finally, in a cohort of patients with normal ejection fraction at increased atherosclerotic risk and/or with stable chest pain, a progressive impairment of 2D global strain and SR (the former with lower variability and higher reproducibility than the latter) was directly related to increasing severity of coronary disease as determined from multislice computed tomography. A global longitudinal strain (the average of segmental longitudinal strains) ≥17.4% provided high sensitivity and specificity (83% and 77%, respectively) in identifying patients with obstructive coronary disease.

Conclusions—Quantitative CTCA improves the prediction of functionally significant coronary lesions compared with visual CTCA assessment but remains insufficient. Functional assessment is still required in lesions of moderate stenosis to accurately detect impaired FFR.

Hypertrophic cardiomyopathy is a relatively common inherited cardiomyopathy that is occasionally challenging to differentiate from hypertensive heart disease and athlete hearts on the basis of morphologic or functional abnormalities alone. Echocardiography has traditionally played a preeminent role in the diagnosis, formulation of management strategies, and the prognostication of this complex disease. In this review, we briefly profile the utility and pitfalls of established echocardiographic modalities and discuss the evolving role of novel echocardiographic imaging modalities such as tissue Doppler, Doppler-based strain, 2-dimensional strain (speckle tracking imaging), and 3-dimensional imaging in the assessment of hypertrophic cardiomyopathy.

Objectives  The goal of the study was to evaluate the prognostic value of echocardiographic indices of right ventricular dysfunction (RVD) for prediction of pulmonary embolism–related 30-day mortality or need for rescue thrombolysis in initially normotensive patients with acute pulmonary embolism (APE).

Conclusions  TAPSE is preferable to the RV/LV ratio for risk stratification in initially normotensive patients with APE. TAPSE ≤15 mm identifies patients with an increased risk of 30-day APE-related mortality, whereas TAPSE >20 mm can be used for identification of a very low-risk group.
