Assessment of dyssynchrony derived from myocardial velocities has limited value in guiding optimization of CRT. In contrast, the simple measurement of stroke distance (LVOT VTI) provides a single, direct measure of global LV function which is robust and easily applicable. It can be applied effectively to the optimization of VV and AV delays in routine clinical practice.

Conclusion
Longitudinal systolic dysfunction maybe be present in hypertensive patients with diastolic dysfunction, despite their normal overall systolic function based on EF.

Altered segmental relaxation patterns correlate highly with longitudinal systolic dysfunction.

A cut-off value of ≤5.9 for Septal Ea appears to associated with concurrent presence of longitudinal systolic dysfunction. LV hypertrophy, SBP and aging are important determinants of both diastolic and longitudinal systolic dysfunction, whereas obesity appears to contribute to the appearance of diastolic dysfunction.

Echo contrast agents are widely used and safe but can rarely produce serious side effects. This—to the author’s knowledge—is the first detailed published case report of a patient who had a severe and complex sequence of adverse reactions within 3 min of having an intravenous infusion of Sonovue initiated, and where the causal connection between Sonovue and the adverse reaction is not diluted by potential side effects from dobutamine.

Conclusion: Aortic arch analysis showed significant pathology in 2% of the adult population undergoing routine TTE. This led to therapeutic interventions in all patients with abnormal findings, and to curative therapy in more than quarter of them. Aortic arch analysis should be mandatory during a routine exam and part of any standard digital acquisition protocol for TTE in adults.
