CMR imaging is emerging as a versatile diagnostic tool for the management of the patient with suspected or established ACS. It provides additional information over other clinical tests for the detection, differential diagnosis, and prognostication after ACS owing to the high spatial definition and multimodal data it provides. Future larger studies will determine more fully the role of CMR in the setting of ACS.

Objectives: The objectives were to compare different Doppler echocardiographic methods for the measurement of prosthetic valve effective orifice area (EOA) following transcatheter aortic valve implantation (TAVI) and to determine the factors influencing the EOA of transcatheter balloon expandable valves.

Conclusions: When estimating the EOA of Edwards-SAPIEN valves by Doppler-echocardiography, it is recommended to use the LVOT diameter and velocity measured immediately proximal to the stent. The main determinant of the EOA of transcatheter valves is the patient’s annulus size and these valves provide excellent hemodynamics even in patients with a small aortic annulus.

Em um caso de prótese aórtica por catéter, notei um gradiente alto e área menor que 1cm2 após o implante.
Mas, claramente, havia a formação de um “túnel” na região valvar.
Isso deixou a dúvida sobre a verdadeira área…
.
Para complementar, o paciente estava muito bem após o implante!

Devo agora fazer como sugere o artigo acima, usando a informação proximal à prótese.
